What to have during your appointment:
- Please know your expected yearly income. Have documentation ready if needed.
- Please have Social Security Number and Date of Births for each member of the taxable household available.
- A list of the doctors you currently see
- A current list of your prescription medications
Note: Other documents maybe required to complete your application. We will talk about those during your meeting (if required)
