What Do I Need to Do Next?

Within the next 24 hours, you will receive an email with a link to your Secure Service Portal.
In your Secure Service Portal, you will:

1. Complete your required Scope of Appointment Form
2. Enter or update your prescription medications
3. Enter the doctors you currently see

Please complete these items as soon as possible. This will give me time to review your information and prepare for our meeting so I can better tailor plan options to your specific needs.

Please arrive 10 minutes before your scheduled appointment.

Directions to Boise Meeting Location- "Medicare AEP 2027"
1. From I-84, take Exit 53 – Vista Avenue.
2. Go south on Vista Avenue.
3. At the first stoplight, turn left onto Elder Street.
4. Go to S. Shoshone Street.  Turn right.
5. The Holiday Inn Express will be on your right.
6. Enter through the main front lobby. Look for the sign directing you to our meeting room.
