What Do I Need to Do Next?

Within the next 24 hours, you will receive an email with a link to your Secure Service Portal.
In your Secure Service Portal, you will:

1. Complete your required Scope of Appointment Form
2. Enter or update your prescription medications
3. Enter the doctors you currently see

Please complete these items as soon as possible. This will give me time to review your information and prepare for our meeting so I can better tailor plan options to your specific needs.

Directions to Caldwell Meeting Location- "Medicare AEP 2027"
1. From I-84, take Exit 29– Franklin Rd.
2. Go West on Franklin Rd.
3. Turn South Specht Ave.
4. On your left you will see Magic Recipe Family Restaurant- turn in to this parking lot
5. Go through the parking lot and you will see the La Quinta Inn.
6. Enter through the main front lobby. Look for the sign directing you to our meeting room.
