What Do I Need to Do Next?

Within the next 24 hours, you will receive an email with a link to your Secure Service Portal.
In your Secure Service Portal, you will:

1. Complete your required Scope of Appointment Form
2. Enter or update your prescription medications
3. Enter the doctors you currently see

Please complete these items as soon as possible. This will give me time to review your information and prepare for our meeting so I can better tailor plan options to your specific needs.

📍 Your Meeting Location

• I will be coming to the address you specified for your Medicare appointment.
• Your appointment is scheduled for the time you selected and will typically last approximately 1½ hours.
• Because I travel between clients' homes, my arrival time may occasionally vary due to travel conditions or the length of a previous appointment. Please be available a little before and after your scheduled appointment time.
• If I am able to arrive significantly earlier than scheduled, or if I expect to be delayed, I will contact you.
