Appointments can be scheduled by phone, Zoom, or at your home; please note that in-home appointments are subject to availability and will be confirmed upon receipt of your request. If you’re unable to find an appointment that works for you, please give our office a call, and we will try to adjust our schedule to accommodate your needs.
If you have not already done so, please complete a Medicare Needs Analysis form so I can assist you effectively during our meeting. CMS requires that we have a Scope of Appointment at a minimum of 48 hours prior to our meeting.
