Once you select a date and time from the calendar below, you’ll move on to Step 2: Complete the Medicare Needs Analysis form.
Please make sure you complete that form. If the form is not received, we can not meet with you. This is a Medicare regulation.
Scheduling Tips:
Friends & Family Group Appointment:
- In-person appointments: Available Monday, Wednesday, Friday, Saturday.
- Phone or Zoom appointments: Available Monday through Saturday.

- Blue dates= Available appointments.
- Grey dates= Sorry, no openings on that day.
- All dates in October are full- please look at November and December to schedule.
- If you can not find a time that works for you, please call my office.

- If friends or family members would like to meet with me as a group, please give me a call rather than scheduling online. If I don't answer, I will return your call as soon as possible.
